Should I attach the Credit Card Authorization form (SB2038) using EFS-Web?

Do not submit Credit Card Authorization forms via EFS-Web. All documents received are electronically processed and scanned into the Image File Wrapper and will be available for viewing. Credit Card authorizations can be made online via EFS-Web without attaching this form. If you do wish to use this form however, please submit via postal mail or by fax.
