(129)        Notice to Customers Making Payment by Check

   Beginning on May 11, 2015, checks sent to the U.S. Patent and Trademark
Office (USPTO) will be converted into an electronic funds transfer (EFT).
This means your check will be imaged and the account information will be
used to electronically debit your account for the amount of the check. The
debit from your account will usually occur within 24 hours, and will be
shown on your regular account statement.

   You will not receive your original check back. Your original check will
be destroyed and a copy will be maintained on file. If the EFT cannot be
processed for technical reasons, the copy will be processed in place of
your original check. If the EFT cannot be completed because of insufficient
funds, the transfer may be attempted up to two times.

   The USPTO is providing information required by the U.S. Department of
the Treasury as follows:

    -  When you provide a check as payment, you authorize us either to
       use information from your check to make a one-time electronic funds
       transfer from your account or to process the payment as a check
       transaction.

    -  Privacy Act - A Privacy Act Statement required by 5 U.S.C. § 552a
       (e)(3) stating our authority for soliciting and collecting the
       information from your check, and explaining the purposes and
       routine uses which will be made of your check information, is
       available from our Internet site at
       (http://www.fms.treas.gov/otcnet/index.html), or call toll free at
       (1-866-945-7920) to obtain a copy by mail. Furnishing the check
       information is voluntary, but a decision not to do so may require
       you to make payment by some other method.
