| 1. An ophthalmic lens, comprising:
an optic having an anterior surface, a posterior surface, and an optical axis;
a first region having a first refractive optical power and comprising (1) a first base curvature having a finite first radius
of curvature and (2) a multifocal phase plate having a first diffraction order with a first diffractive optical power, the
first region configured for forming a first focus and a second focus; and
a second region having a second refractive optical power and comprising (1) a second base curvature having a finite second
radius of curvature different from the first radius of curvature and (2) a monofocal phase plate having a first diffraction
order with the second diffractive optical power, the second region configured for forming a third focus.
