1. An anchor assembly configured to couple a bone anchor to a spinal rod to stabilize bones or bone fragments, the anchor assembly comprising:
a body having an upper end that defines an upper opening, and a lower end that defines a lower opening and that is spaced from the upper end, the body defining a bore that extends substantially between the upper opening and the lower opening, and defining a first rod-receiving channel configured to receive the spinal rod;
a sleeve configured to be disposed in the bore of the body, the sleeve including at least one arm that extends along a longitudinal axis of the bore and defines a portion of a second rod-receiving channel configured to receive the spinal rod;
an insert member configured to be disposed in the bore of the body, the insert member including an upper end portion and a lower end portion, and the insert member defining an interior cavity that extends into the lower end portion towards the upper end portion and that is configured to receive at least a portion of a head of the bone anchor, the insert member defining at least one slot extending from the lower end portion towards the upper end portion so that the lower end portion is expandable and collapsible so that the head of the bone anchor can be inserted into the interior cavity of the insert member and locked in the interior cavity; and
at least one movable element, the at least one movable element being configured to be received within the bore of the body to
wherein the at least one movable element is engagable with the body and movable along the longitudinal axis from a first position to a second position,
wherein the at least one arm includes an end that is configured to engage the movable element such that movement of the movable element from the first position to the second position urges the sleeve along the longitudinal axis,
wherein the sleeve is configured to urge the insert member from an unlocked position to a locked position, when 1) the head of the bone anchor is disposed in the interior cavity of the insert member, and 2) the movable element subsequently moves from the first position to the second position, and
wherein the unlocked position, the bone anchor is poly-axially rotatable with respect to the body, and in the locked position, the insert member secures a position of the bone anchor with respect to the body.
