1. A computer-implemented method, comprising:
accessing a first set of patient data associated with a first patient and a second set of patient data associated with the first patient;
determining first score information based on the first set of patient data corresponding to a first time;
determining second score information based on the second set of patient data corresponding to a second time that is subsequent to the first time;
detecting a trend associated with a change between the first score information and the second score information;
assigning a first level of risk for the first patient based on the trend; and
based on the first level of risk, outputting a warning or notification associated with the first patient for an interface.
