Procedures for determining adjustment:
Specific information required in a requests for reconsideration:
- The correct patent term adjustment
- The bases for the adjustment
- The relevant dates for which an adjustment is sought and the adjustment to which the patent is entitled
- Whether the patent is subject to a terminal disclaimer (and date in the disclaimer)
- Any circumstances that constitute a failure to engage in reasonable efforts to conclude processing or examination of such application (or a statement that there were no such circumstances)
