HOW TO USE THE FILLABLE ADOBE PDF ORDER FORMTo use this fillable Adobe PDF PTCS order form, you must have the free Adobe Acrobat Reader version 3.0 or later installed on your PC or Mac. Adobe Acrobat Readers for other platforms, such as UNIX, do not support PDF forms. See the Adobe site for details. [Click here to get the free Adobe Acrobat Reader.] Clicking on the link to the form will bring the form up either in a separate Acrobat window, or within the browser window, depending upon your installation. Once the form is displayed, the cursor will be positioned in the first fillable field. Anything you type will be entered in that field. Moving to another field may be accomplished by either clicking on the field with the mouse, or by using the tab or shift-tab keys. If the field is a check box, clicking on the field with the mouse will place a checkmark in the box. Please insure that you have checked the correct boxes to indicate the methods of delivery and payment you have chosen. Enter data in all the fields necessary to fully specify information about you, the customer; the method you wish used for delivery of the documents you are ordering; everything required for your method of payment; each document you wish to order; any optional delivery instructions; and any comments you might wish to offer. You may submit the form via Internet email simply by clicking on the "Click Here..." box outlined in green at the top of the form. Such electronic submission requires that you have entered at least the minimum, mandatory data fields outlined in red; specifically, the required User Information, at least one ordered document number, and your return email address at the bottom of the form. -- OR-- You may now print the filled-out form using the print function of either your browser or the Acrobat Reader, as appropriate, and send the form (with a payment instrument, if necessary) or fax the form to PTCS at the address on the Copy Sales page. It is also possible to use the features of your browser (usually either a right mouse click "Save Link As", or the menu's "File" -- "Save As") to save the blank fillable form on your hard disk. You may then use the Acrobat Reader off-line to fill out and submit as many orders as you like in the future. |
