2. Trademark/Servicemark Application, Supplemental Register

Use this form to file an initial application to register a trademark/servicemark on the Supplemental Register, rather than the Principal Register. Certain marks that are not eligible for registration on the Principal Register may be registered on the Supplemental Register. For more information on the Supplemental Register, see Trademark Manual of Examining Procedure Section 815 et. seq.

NOTE: Before filing an application requesting registration on the Supplemental Register, you must be lawfully using the mark in commerce on or in connection with the identified goods and/or services (unlike an application on the Principal Register, which may be based on an intent to use the mark in the future, under Section 1(b) of the Trademark Act). The only exception from the use requirement in seeking registration on the Supplemental Register is for applications based solely on Section 44 of the Trademark Act (i.e., based on a foreign application and/or registration).

3. Certification Mark Application, Principal Register

Use this form to file an initial application to register a certification mark, which is a mark that is used or intended to be used by authorized parties to show that goods or services have been certified, namely, to meet certain qualifications and standards. That is, a certification mark certifies regional or other origin of goods or services, as well as certifies other characteristics of goods or services, or certifies that the work or labor for the goods or services was performed by a particular group). A copy of the certification standards will be required. 4. Collective Membership Mark Application, Principal Register 

Use this form to file an initial application to register a collective membership mark, which is a mark used by the members of a particular organization to signify that they are members of that organization (e.g., a social club or labor union). The applicant will be required to explain how it controls use of the mark by its members. 5. Collective Trademark/Servicemark Application, Principal Register

Use this form to file an initial application to register a collective trademark/service mark, which is a mark used by members of a collective organization to identify their goods and/or services and distinguish them from the goods and/or services of non-members. The applicant will be required to explain how it controls use of the mark by its members.
