How do I apply for a customer number?

Go to the Patent EBC website and download the Request for Customer Number form.

Fax the form to 571-273-0177 or mail it to

Mail Stop EBC,
P.O. Box 1450 ,
Alexandria VA 22313

Depending upon the set-up for each firm or corporation, there must be at least one customer number for each correspondence address. A customer number cannot be associated with more than one address.
