What information do I need to provide to the EPO to get them to retrieve a priority document of a US application for placement in my European patent application?

The retrieval of a US application (priority document) is automatic in the event the applicant validly claims priority. This applies to European patent applications filed on/after 16 January 2007. Please contact the EPO for further information.
