Survey of Registered Practitioners in Patent Cases
The Office of Enrollment and Discipline periodically conducts a mandatory survey of active registered practitioners pursuant to 37 C.F.R. § 11.11. The purpose of the survey is to ascertain whether the practitioners desire to remain on the register. The United States Patent and Trademark Office benefits from the survey by obtaining confirmation that OED's records regarding each practitioner's address and telephone number are correct, or by receiving updated information. Practitioners who fail to respond may be subject to administrative suspension.
Lack of Response to Survey by Registered Practitioners in Patent Cases
Pursuant to 37 C.F.R. § 10.11, the Office of Enrollment and Discipline (OED) conducted a survey of the active registered practitioners whose registrations numbers are below 25,000. The purpose of the survey, begun on July 31, 2012, was to ascertain whether these practitioners wish to remain on the register. OED received numerous responses. Most practitioners responding to the survey provided information that enabled OED to confirm and update records regarding the responding practitioner’s address and telephone number. Other practitioners requested removal of their names from the register of attorneys and agents. As of December 12, 2012, OED did not receive any response or communication from over 900 practitioners. Removal of Registered Practitioners
Practitioners who failed to reply and give the requested information to the Survey were previously removed from the register. Click here for more information.
Practitioners who have been administratively removed from the Roster of Patent Attorneys and Agents may apply for reinstatement by completing the Reinstatement Data Sheet (FORM PTO 107R) and submitting the $100.00 for reinstatement processing. 37 CFR 1.21(a)(9)(ii)

Practitioners whose names have been removed from the Roster of Patent Attorneys and Agents for more than five years must, in addition to completing and filing the Reinstatement Data Sheet [FORM PTO 107R] and submitting the $100.00 for reinstatement processing, take and pass the registration examination unless they submit a showing to the satisfaction of the Director of Enrollment and Discipline that they continue to possess the legal qualifications necessary to render patent applicants valuable service. Rule to Show Cause for Survey of Registered Practitioners
Pursuant to 37 C.F.R. § 10.11 (now 37 C.F.R. § 11.11(a)(2)), the Office of Enrollment and Discipline (OED) conducted a survey of the active registered practitioners whose registration numbers are between 30,000 and 34,999.
As of November 5, 2013, OED has not received any response from the practitioners on the list below. They are hereby given 30 days to Show Cause why they should not be administratively suspended from practice in patent cases before the USPTO. Practitioners on the list below who fail to reply and provide the requested information will be subject to administrative suspension.

Practitioners who have information that a former colleague, whose name appears on the list below, may be deceased are requested to inform OED.
